Below is a sample of the Export Group Selection Screen.  When a particular button (indicating a type of data) is pressed, a selection screen will appear as shown directly below this screen sample.

When the Include or Exclude radio button is pressed for a certain data field, a pick list is displayed that allows the user to indicate which values should be included or excluded.  Below is a sample of the state selection list.
